Your initial appointment will consist of an examination and consultation explaining your diagnosis and treatment options. Occasionally, surgery can be done the same day as the consultation. However, a complex medical history or treatment plan will require a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
• Your surgical referral slip and any x-rays if applicable.
• A list of all medications and supplements you are currently taking.
• Any medical and dental insurance cards.
• A valid driver’s license or photo identification
IMPORTANT: Patients under 18 years of age must be accompanied by a parent or legal guardian at both the consultation visit and surgery appointment.
A pre-operative consultation and physical examination is mandatory for patients undergoing any surgery. If you have a consultation scheduled and hope to have your procedure that same day as well as wish to possibly undergo I.V. anesthesia.
Please alert the office if you have a medical condition that may be of concern prior to your surgery (i.e. diabetes, high blood pressure, artificial heart valves or prosthetic joints, history of rheumatic fever, etc.) or if you are on any medication or supplement (i.e. heart medication, blood pressure medication, diabetes medication, blood thinner, etc.). A registered nurse may call you prior to your appointment to review your medical history and medications.
X-Rays
If your dentist or physician has taken x-rays or scans pertinent to your planned consultation, please request that these be forwarded to our office. If there is not enough time for them to arrive for your appointment, please pick them up and bring them with you the day of your visit. If additional x-rays or scans are required, they can be made at our facility.
